Full-Time Home Health Physical Therapist
Grey to Glory Home Health is seeking a compassionate, motivated Full-Time Physical Therapist to provide exceptional in-home therapy services to patients throughout our service area. The Physical Therapist is responsible for evaluating patients, developing individualized treatment plans, implementing therapeutic interventions, and collaborating with the interdisciplinary team to help patients regain mobility, improve function, and maximize independence in the home setting.
Responsibilities:
Perform comprehensive physical therapy evaluations and reassessments.
Develop and implement individualized treatment plans.
Provide skilled therapeutic interventions to improve strength, balance, mobility, and safety.
Educate patients, caregivers, and families on exercises, fall prevention, and home safety.
Complete accurate and timely documentation in accordance with Medicare and agency guidelines.
Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
Participate in care planning and quality improvement initiatives.
Qualifications:
Current Florida Physical Therapist license.
Current CPR certification.
Valid driver's license, reliable transportation, and auto insurance.
Home health experience preferred but not required.
Excellent communication, organizational, and clinical skills.

Requirements
Home Health Physical Therapist – Minimum Requirements (Florida)
Current, unrestricted Florida Physical Therapist (PT) license
Graduate of an accredited Physical Therapy program (DPT or equivalent)
Current CPR/BLS certification
Valid Florida driver's license
Reliable transportation with current automobile insurance
Ability to travel throughout the assigned service area for home visits
Excellent assessment, clinical, and documentation skills
Strong communication and patient education abilities
Ability to work independently while collaborating with an interdisciplinary healthcare team
Knowledge of Medicare Conditions of Participation and home health regulations preferred
Home health experience preferred, but willing to train the right candidate
Must successfully complete background screening and meet agency employment requirements
